If you do not have Adobe Reader, you can download it here for several different platforms, including Windows and Macintosh. On these applications, you can use Adobe Reader/Acrobat Reader to electronically fill in the form and then print it out to fax back. Don't forget to sign the application before you fax it! Note that you will NOT be able to save the document after you have typed out the necessary information (as Adobe Reader does not allow this).
